This form needs to be completed if you or your relative has a substantial or remote interest in a company that does business with KUSD. It is designed to protect employees and the district. The document states that if a conflict of interest exists, you will refrain from participating in any manner as a public employee.

Ex.) Your brother-in-law works for ABC company and ABC company provides services to KUSD. This relationship needs to be disclosed, and as a public employee, you would not be able to participate in any manner with ABC company.
