Section 504 Grievance Procedures

The Kingman Unified School District #20 (KUSD) grievance procedure is intended to provide prompt and equitable resolution of complaints alleging action which is prohibited by the Americans with Disabilities Act (ADA) and Office of Civil Rights (OCR) regulations implementing Section 504 of the Rehabilitation Act of 1973 and the ADA Amendments Act of 2008.

When an employee, student, or parent of a student, believes discrimination is occurring due to a disability, the individual may follow the process below.

Mrs. Trujillo is the district coordinator for Section 504.

The complaint should be filed in writing when possible, although verbal complaints can be scribed at the KUSD district office at 3033 MacDonald Avenue in Kingman, Arizona. The complaint should include the name, address and phone number of the individual filing the complaint. It should briefly describe the alleged violation of the regulations.

The complaint should be filed within fifteen days of the date the complainant became aware of the alleged violation.

Investigation, as appropriate, will follow the filing of the complaint and will be conducted by Mrs. Gretchen Dorner, Superintendent, or her designee. An informal but thorough investigation will be completed and all interested persons, or their representatives, will have an opportunity to submit evidence relevant to the complaint.

A written determination including a description of the resolution relative to the complaint will be issued by Vicki Trujillo, 504 Coordinator, and a copy mailed, e-mailed or hand-delivered no later than thirty days from the date KUSD received the complaint.

Allegations substantiated by investigation as above will be corrected by KUSD and procedures, trainings, or other corrections will be put into place to prevent recurrence.
