Checklist for College Bound Seniors

Former K.H.S. students who are in need of their transcript may obtain them through the Guidance Office.  In order to protect our student’s privacy, we don’t accept verbal request.  If you still live in the area, you (student or parent listed on the transcript only) may come to the Guidance Office during office hours to make your request.  Please bring your photo ID and we will process the documents.
If you are living out of area, we require that you write a letter including the following info.:  name at time of graduation/withdrawal, birth date, date of graduation/withdrawal and the address you want the official transcript to be mailed to.  Please mail the letter to the school along with a clear copy of your photo ID to: K.H.S. Attn: Guidance Office 4182 N. Bank St. Kingman, AZ  86409.  You can also fax the letter along with a clear copy of your photo ID to: 928-692-1507.  Please note that an official transcript can’t be faxed.  There is no charge for this service!!
